Bridge to Wellness

Beginning October 1st, 2018, we have a brand new points-based incentive program that all benefits-eligible active employees are welcome and encouraged to participate in.  You can read all about the new tiered system in the Welcome letter HERE and find your tracking form HERE.  There will soon be a link on this site which you will be able to actually REGISTER yourself for the program.  When that is up and running a notice will be sent out so that you may sign up. Until then - read over the material and feel free to start collecting points! The program runs Oct 1, 2018 -Sept 30, 2019, which means that if you had a physical for example, in September of 2018, you will have to make sure you set up your appointment for next Sept 2019 to earn those points.

LAKE SUPERIOR (BAYFIELD FOODS) CSA SPECIAL ORDERS OPEN FOR 2019-2020 SHARES! - First of all for those of you who are new to this, CSA stands for Community Supported Agriculture. You’ll receive the freshest and tastiest produce you can ever imagine through a CSA (unless you have your own garden, and then it’s hard to compare), along with the option to purchase meats, baked goods and other locally sourced items.  The best part (in my opinion) is that you are supporting local farmers who use organic methods to raise their crops/animals/etc. Clean, real food.

The City of Duluth is a drop-site for the Bayfield Foods/Lake Superior CSA and if you choose to order, you will be able to pick up your food box from City Hall on a weekly (or bi-weekly) basis.  . To learn more or place and order check out Bayfield Foods

Bruce and Roxy Schmidt operate a Community Supported Agriculture farm in Willow River, MN.  Bruce was out at the Farmer's Market this past summer.  He and Roxy have a simple form to use for their CSA share.  THIS FORM includes all of the produce/goods you can choose from, their contact information, pick-up location and more.  Please contact Angel if you have any questions at or 218-730-5201.

 We are here to support you.  If you are contemplating quitting tobacco, there are many FREE resources available to help you:

 If you have questions about any of these services or would like additional information, please feel free to contact Wellness Coordinator, Angel Hohenstein at or 730-5201

Am I at Risk for Prediabetes and Type 2 Diabetes?

You are at increased risk for developing prediabetes and type 2 diabetes if you:

  • Are 45 years of age or older;

  • Have a family history of type 2 diabetes;
  • Are physically active fewer than three times per week; or
